The limb lengthening procedure is divided into two parts: a lengthening period and a hardening period (consolidation period).
In a conventional Ilizarov method, the external fixator (Ilizarov) must remain attached for the entire period. It causes pain and many complications. The LON technique was invented to minimize the period of external fixator attachment. To do this, two operations are needed in LON.
For the first operation, an intramedullary nail is inserted into bone marrow cavity and external fixators are attached. After the surgery, the patient lengthens the bone slowly using the external fixators. When lengthening is complete, a second operation is performed to fix the internal fixator with screws (to preserve the length) and remove the external fixators.
Since the internal fixator holds the bone during the hardening period, the period of external fixator attachment is reduced to almost half of that with the conventional technique.
Lengthening Over Nail Procedure
Dr. Donghoon Lee’s technique is different from conventional LON. He addressed the shortcomings of the conventional LON method by inventing two important techniques to optimize results.
